We try our best to stay on schedule to minimize your wait time. Due to the fact our doctors provide many types of services, various circumstances may lengthen the time allocated for a visit. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

As a new patient to the practice, here are some things to prepare for your visit.

1. In an effort to minimize your time in the office, we ask that you fill out the patient registration forms and medical history by creating your patient portal account using the secure link below.

The most effective and secure form of communication with our office and to view information and test results is through the patient portal. Patient portals are sanctioned, secure and private means of commmunictaion between providers and patients. Completing the account activation is quick and easy. You will receive and email with a temporary password to set up and register. You can also register directly through the link below. The patient portal allows 24/7 access to the practice, appointments, records, etc.

An alternative to the submitting information through the patient portal should you choose not to, you can also fill out the forms on our website through this Online Registration link. The forms can be submitted directly throught the website or you can print them out and bring the signed forms with you to your office visit. You can also contact us to receive an email to return the documents through an encrypted service.

2. Return all forms with a copy of the front and back of your insurance and identification cards so that we may run your insurance eligibility prior to your visit to minimize your wait time.

3. Bring your glasses and contact lenses box labels if you wear any.

4. We continue to observe strict safety precautions to protect our patients as well as our staff including:

  • filling up our COVID-19 Questionnaire before coming to the office
  • requiring masks be worn by all people at all times
  • enforcing social distancing in waiting areas
  • checking temperatures of all staff and patients that comes to the office
  • sanitizing and cleaning all surfaces throughout the day
  • nightly sanitation using electrostatic fogging throughout the office
  • continuous air filter with UV lighting installation in our air conditioning units
  • limiting the number of people in the office at the same time by asking that you do not bring anyone with you unless you need assistance (aide, translator, etc), and to not arrive too early.
